Our Aim: ‘Facilitate the development and implementation of Healthcare solutions to promote ‘whole well-being’ Our Objectives: • Maximising the impact of the Third Sector on Health in the West Midlands. • Supporting early intervention, health promotion and prevention of ill-health/Long Term Conditions • Reducing health inequalities • Improving overall health status Achievements to date: (brief resume including current projects and initiatives) Well established networks within the third sector, community and faith groups; Various action research projects undertaken including community based models of self care and management of long term conditions using QOF data and Experts by Experience initiatives; a number of stand alone projects and assignments; Awarded the contract to manage A GP-Lead Service in partnership with George Eliot Hospital NHS Trust. CatalystPlus is focusing on the delivery of primary care in the community via the creation of a Health Xchange Hub which will include a Health Forum, 50+ Clubs, Enterprise Clubs, Health and Fitness Centre, etc. (Key Performance Indicators include: Low level support for people with mental health problems; Young people - sexual health, Respite care for carers; Older adults; Pharmacy advice; Screening for cervical and breast cancer); Developed and supporting a number of regeneration including environmental and health projects; working with various faiths groups to address identified needs. Undertaken various research projects such as assessing the capacity of the Third Sector to manage NHS contracts. Support 2 Contract, Routes 2 Opportunities, BME Older People, Micro-Clusters (pilots) and various others. The above is not an exhaustive list of achievements.
